Understanding your Insurance Benefits
For the most positive experience utilizing insurance, please understand your insurance benefits. Call your insurance company with any insurance questions, and ask them if you can see out-of-network providers.
We will happily process out-of-network claim submission, helping you utilize your benefits.
After your appointment, we will submit your claim to your insurance company. If you have a deductible or co-pay for services provided, those fees are due in full at time of service.
We do our best not surprise you and will help you understand your insurance benefits. For your convenience, we can send a predetermination to your insurance company prior to any treatment. This will explain what insurance helps cover and the estimated out-of-pocket expense which you are responsible for. Please understand that treatment can change and that you are ultimately responsible for all fees for your appointment. It is best that you contact your employer or insurance company with questions in regard to your benefits.
Payment Options
Payment is due in full at the time of service, for this reason we have payment options. For services not covered by insurance, we accept cash, check, or major credit cards to help achieve your best smile.
We offer a 10% discount to those paying by cash or check.
We are not credentialed to accept Forward Health or Badger Care.
Patient Policy
Our patient policy is a required at least 24-hour notice for cancelling or rescheduling appointments. At Govani Dental we set aside time and have reserved it especially for you. A $75 fee will be applied to your account for rescheduling, cancelling or failing to show up to your appointment without the required 24-hour notice.
